2. Aim Statement  Our current rate of closing the loop with specialty labs is 75% within 30 business days. Our Aim is to close 90% within 30 business days by 6/30/2016.  This is important for Patients Safety because patients are confused by directions, complexity, and when to expect results with specialty labs  This improves the patient experience as the testing process will be fully explained prior to receiving the test kit. 2

4. What we are Measuring  Keep a list of specialty labs ordered starting Tuesday 3/1/2016. Measure how many were closed by 5pm on Friday 4/1/2016.  Measure patient satisfaction in process and record reaction to changes we are testing. Obtain patient feedback for further improvement 4
