GUIDELINES 1. Groupings are based per Work Teams. Groups will create their own Oplan names. 2. The team must not violate the presented rules. 3. There will be random check from the Admin group. 4. If member of your team violated any of the rules, the team shall have ONE DEMERIT (P5.00) 5. If a member of your team reported an incident of someone who violated any of the rules, his team will earn ONE MERIT. 6. ONE MERIT = ONE DEMERIT DEDUCTION ONE DEMERIT = 5 PESOS DEMERIT – MERIT = GREEN FUND

GUIDELINES 7. The group must have its own strategy on how to raise their funds if ever they need to pay for the demerits. 8. The group will assign their Planet Ambassador to whom Admin group will collect the green fund based on the accrued penalty every 2 nd pay day of the month. 9. Teams’ statistics will be posted to the bulletin board and HR website. 10. At the end of the each quarter, the 70% of the fund will be given to the team who has the least number of demerits and 30% to the Admin group. 11. This shall be in effect starting February 10, 2014. NOTES: Only tallies are presented, name of the violators are documented, name of the reporter will not be disclosed.
