Responsible Clinician: is responsible for the care and treatment of your family/friend Allocated a named nurse: who will coordinate your family or friends care. Carer champion: who will be available to speak to you. The weekly care package and progress will be reviewed weekly by the MDT Care Programme Approach (CPA) meeting within 3 months to review the whole care package and plan for the next period of care CPA every 6 months Prior to discharge – planning meeting
