What is Employment and Community First CHOICES? A program called “ECF” for short A NEW community supports program for people with An intellectual disability or A developmental disability

Who is Employment and Community First CHOICES (ECF) for? For people with intellectual disabilities not already getting services through the Department on Intellectual and Developmental Disabilities (DIDD) “waivers” For people with developmental disabilities – this is the first “waiver” program that will serve them

When will Employment and Community First CHOICES (ECF) start? Starting July 1, 2016 Statewide Plan to serve a total of 1,700 people in the first year

Who will ECF serve in the first year? Plan to serve 500 who are living with their families: Children under 21 Adults age 21 and older Plan to serve 1,000 who are: Adults age 21 and older At risk of going into an institution if don’t have these supports Plan to serve 200 adults who are: Currently living with aging family caregivers Could qualify to go into an institution

What is the Goal of ECF? Why was ECF created? To put Employment First and Community First when supporting people with intellectual and developmental disabilities to have a good life in their chosen communities.

What are the Goals for Each Person in ECF? A good life in his or her community including: A job the person enjoys that helps pay the bills and involves work that is important to the community A home and home life that the person chooses A lifestyle that the person chooses A chance to be involved in the life of their community

What are the Goals for Each Person in ECF? A good life in his or her community including: A chance to be known and appreciated by others who live in the same community A chance to be as independent as possible – to do and achieve things other people might have thought wasn’t possible. A chance to have relationships with people who care and who are not paid to care

What are the Goals for People in ECF? To help families stay together and stay strong To help children and youth with intellectual and developmental disabilities to believe they can graduate from high school and get a job To help people have the best possible health To prevent crisis

Who will run the Employment and Community CHOICES Program? Two of the three Insurance Companies that currently provide physical and mental health coverage to people with intellectual and developmental disabilities on Medicaid Amerigroup and Blue Care United will become involved when the program expands

Why Use Insurance Companies and a “Managed Care” Model? One agency making sure all of the medical services and community support services work together for a person. To make sure people get the right type of services, in the right amount and at the right time. To spend less per person - while still serving people well – and by spending less, being able to serve more people

What Services Will Be Available Under ECF? Employment Services Exploration Discovery Benefits counseling Situational observation and assessment Job development plan or self-employment plan Job development or self-employment start up Job coaching for Individualized, integrated employment Job coaching for Individualized, integrated self-employment Coworker supports Career advancement Supported employment – small group Integrated employment path services

What Services Will Be Available Under ECF? Other Services to Support Community Involvement and Independence Community integration support services Community transportation Independent living skills training Personal assistance Assistive technology, adaptive equipment and supplies Specialized consultation and training Minor home modifications Community living supports (CLS) and CLS – family model Individual education and training Peer-to-peer Support and Navigation (for Person Centered Planning, Self-Direction, Integrated Employment/Self- Employment and Independent Community Living)

What Services will Support Family Caregivers? Supportive home care or Family caregiver stipend Respite Family caregiver education and training Conservatorship and alternatives to conservatorship counseling and assistance Health insurance counseling/forms assistance Community support development, organization and navigation Family to Family Support 14

Support Coordinators Everyone in ECF will have a Support Coordinator In ECF, support coordinators will work for the Insurance Companies Support Coordinators will: Get to know a lot about the person through a guided assessment process 16

Support Coordinators A Support Coordinator helps people decide many things: The kind of lifestyle they want to lead The place they want to live The people they want to live with The other people they want to have in their life What kind of employment they want to have The other ways they want to be involved in their community The things they want to learn The ways they want to be more independent 17

Support Coordinators With all of the information learned from the person and others who know the person, the Support Coordinator will: Develop the “Person Centered Support Plan” along with the person and others important to the person The “Person Centered Support Plan” will list the person’s lifestyle choices, goals and support needs The “Person Centered Support Plan will also list the services that can help the person have the life she/he wants and achieve his/her goals 18

Support Coordinators The Support Coordinator will also: Provide information about service providers that can be chosen Explain which service providers are “preferred” providers because they have met higher standards Ensure the providers that are chosen by the person have the information they need to support the person Keep in touch with person, other people offering support, and the paid providers to make sure everything is going well and to fix any problems 19
