2 Supporting carers in Nottingham Importance of carers – Early intervention and prevention Many carers don’t recognise that they are carers ALL carers are entitled to an assessment of their own needs We can only offer carers assessments and support if we can find them! Support for Nottingham’s most vulnerable citizens Prevent escalation of need in the people they care for. Carers’ quality of life improved through early identification, assessment and support. We can often find carers through the patients they care for.

3 Identifying carers Every single patient you see may have a carer! Does the carer come to appointments with the patient? Could they be invited? Ask the patient Who else looks after you? Could I speak to them? Children can be carers too! Please refer carers for support
