8 Prevents duplication and seeks child, young person and family view at earliest opportunity. Identify resources within community this is not necessarily statutory services. Brixham Hub has capacity to signpost to services and support families through this process and into the future ( community resilience ). The Orb. Think outside the box –what would make a difference. 9 Acts as single point of contact for child young person or family. Leads the process of putting one clear plan in place with families views being central. Ensure progress is being made. Keeps families involved and informed. Makes sure others involved in plan are clear about their responsibilities. Develops a professional relationship with child/young person /family. 10 To look at identified needs that will require support /interventions from more than one agency. Seek to address identified and unmet needs in partnership with the family. Agree to appoint a Lead Professional if not already appointed Centred on the needs and engagement of the child/young person and their family. This is an inclusive and consented process. Team Around the family Meeting 10

11 Team Around the Family Advisor Promote, empower and support partners. They will be based in a Community setting (e.g. The Barn, Brixham) They will support training and professional development, and provide solution focused case consultancy for complex cases, where cases are not progressing, or where there is professional dispute. They will facilitate direct referrals into the community where the cases do not warrant a Team Around the Family approach and to ensure TAF plans are recorded on the appropriate systems. They will be part of the Early Help Service (and be managed by the Early Help coordinator), and work closely with the Supporting families team, and the SWIFT team. They will make decisions on who should be involved in the TAF and will need to challenge families and Partner agencies and services around non-engagement and reluctance to work as LP. 11
