Preparing CQI Action Plans by 31 January 2016 Powerpoint Tool – Life Cycle Interventions to assist PDSA Cycle Analysis 31 December 2015

CQI Action Plans of ACCHOs – PDSA Cycles Member ACCHOs of NACCHO are required to submit “CQI Action Plans” to the Commonwealth Department of Health by 31 January 2016 In August 2015, the Department issued a template which was “developed to align with the core components in the draft National CQI Framework for Aboriginal and Torres Strait Islander Primary Health Care that are relevant at the local service level.” There are 9 of these core components. In an accompanying “Guide to developing the Action Plan”, the Department gave examples of what to put in to their template The 9 th of these “core components” is PDSA Cycles, and the “Guide” says: “ PDSA Cycles: Aim: The organisation has developed strategies to ensure rapid PDSA cycles are supported and ongoing throughout the organisation” The “Guide” suggests that ACCHOs: “ Establish a process for identifying priority areas for PDSA and embed PDSA cycles” To assist Member ACCHOs apply PDSA Cycles, the NACCHO Secretariat has developed a Map which identifies the priority areas of health care throughout the life cycle of a patient. Your ACCHO could apply the PDSA Cycle methodology to any of these priority areas in the Map as you consider appropriate for your Service. Mapping the Patient Journey through Life Cycle Interventions NACCHO has developed a Map that diagrams a patient’s life cycle journey from pre- conception through to death. Please refer to the following Powerpoint. The journey starts in the centre left hand side with “Reproductive health, education, healthy living, STI screening”. Then comes the line dealing with “Education, Awareness” during pregnancy and early childhood. Then the Map goes on with “Promotion, Prevention” through to young adulthood. Finally the Map outlines the stages of adult health, aged care, culminating in death. At every stage in the patient’s journey, there are services in red colour as examples of interventions, support and treatment by ACCHOs. NACCHO hopes that this Map provides ACCHO CEOs, Practice Managers, Senior Medical Officers and Population Health Managers with a useful overview of where your ACCHO could apply a PDSA Cycle to the benefit of your patients as part of your CQI Action Plan
