4. Log Book Completion Guidelines Refer to page 5 of Practical Training Log This log book is made up of six sections. They are as follows: Section 1:Student and training details 2:Firm’s details 3:Weekly summary of training 4:Final summary of training 5:Student evaluation form 6:Firm’s evaluation form 7:Guidelines for the practical training report.

6. Final report preparation Refer to page 15-18 of your Practical Training Log

7. Appendix:  Refer to page 6 to 11: section 1 to 5  Refer to page 12 to 14: section 6 (employer’s evaluation form) Final report preparation

8. Weekly Report Refer to the sample Complete week by week Send to supervisor on time Mode of submission depends on the supervisor Have to attach all the original copies of your weekly report – signed by your employer and attach them as the appendix of your final report.

9. Employer’s evaluation form Refer to page 12 to 14 of Practical Training Log: section 6 Prepare a copy of ‘employer’s evaluation form’ for your employer to fill up before the visitation of your supervisor. After complete the form, please ask your employer place the form inside a sealed envelope and pass it to your supervisor on the day of visitation.

10. Practical Training Visitation Supervisor will make an appointment with student regarding the day of visitation. Please prepare a location map of your working place and send it to your supervisor before the visitation.

11. Reminder Final Report Submission : Due on 15 September 2011 (before 5pm) Please check the supervisor_student list. Update Ms.Azleen or Ms. Nurbani when necessary.
