2 Problems Taking charge of more casualties in radiological controlled area   First aid workers with specific training   Secondly, hospital and firemen emergency services No radiation protection training for emergency services Mixed injuries : conventional and radiological External exposure with or without contamination, internal exposure

3 Stakes Give priority to taking charge of casualties and conventional injuries treatment Ensure radiation protection of all contributors Avoid causing anxiety towards hospital emergency services

4 Organisational Principles Depends on the number of casualties and the seriousness of their injuries If necessary, organization like disaster medicine Local activation of an Internal Emergency Plan of a Sanitary nature Medical radiation protection support from the site medical service

5 Scene of the Accident Scene of the Accident Evaluation of the radiological risk accident area and marking high radioactivity points and contaminated zones Estimation and optimisation of radiological exposure for casualties and first aid workers

6 Scene of the Accident Ensure protection for casualties and first-aid workers Check the efficiency of all the taken measures Record the radiological conditions suffered External exposure Internal exposure

10 Means of transport Two « noria » : One for contaminated casualties One for non contaminated casualties Ensure : radiation protection during transport Control of the staff and material at the end of the emergency operations

11 Reception at the Hospital Principle of unity of place No spread of contamination Principle of unity of time Decontamination as soon as possible Principle of unity of action Hospital staff and radio-protection teams

12 Conclusions (1) Probability of a major accident with a lot of casualties is quite uncertain At the opposite, probability to have one contaminated injuried casualty in RCA is important

13 13 Conclusions (2) Conclusions (2) Keys of success are : Support of the NPP radiation protection teams during all the emergency rescue operations Hospital and firemen staff training Regular exercices in NPP with simulated accident Maintening a trustful relationship with the external emergency services.
