Falk Pharmacy Options for Patients › Patients receive prescription for immunizations and schedule appointments to receive on campus › Patients may call Falk to request assistance and prescription for immunizations cay be obtained through the immunization protocol › Physician Advocate/Sponsor – Dr. Thuy Bui Immunizing Pharmacists › Currently 9 pharmacists licensed to immunize at Falk Hours of operation at Falk Pharmacy: › Monday – Friday 7:30am – 5:30pm

To protect individuals over 18 years of age from preventable infectious diseases that cause needless death and disease according to indications and contraindications currently recommended by the CDC. Provide a convenient service for UPMC employees and the community.

Pharmacist › Access appropriateness of vaccination including indication, drug interactions, contraindications, and timing › Provide the immunization › Monitor patient for a minimum of 15 minutes after administering the immunization › Initiate emergency response if necessary in event of anaphylaxis › Document vaccination and route to physician in EPICare within 48 hours Patient › Schedule initial appointment with Falk Pharmacy to determine which immunization needs to be provided and review all current medications › Will pay cash prices for travel immunizations
