1 Developing a Resource-Based Relative Value Scale system for traditional medicine services in the Korean National Health Insurance Jin-Hyun Kim, Byungmook Lim * College of Nursing, Seoul National University, 28 Yongon-dong, Jongno-gu, Seoul, South Korea, *Division of Humanities and Social medicine, School of Korean Medicine, Pusan National University, Mulgeum, Yangsan-si, Gyeongnam, South Korea  This study was to identify resources invested into clinical activities and to develop a new RBRVS for Korean medicine services as part of the National Health Insurance. Objectives  The RBRVS consists of the physician’s work(PW) and the practice expense(PE). For each procedure of traditional Korean medicine, PW and PE were estimated separately and then were put together to calculate total RBRVS. We compared the RBRVS scores with the current fee schedule for reimbursement in the National Health Insurance.  PW : Based on a survey of physicians' time, physical effort and technical skill, mental effort & judgment, and stress that were used or experienced during patient treatments, the relative input values for the relevant clinical activities were estimated. We used the Magnitude-estimation Method, and the reference procedure was the acupuncture treatment for the patient with mild ankle sprain.  PE : We collected the cost date from the sample Korean medicine clinics. The cost data included the depreciation cost of devices for diagnosis and treatments, expendable supplies, and the input time and salaries of clinical assistants(eg. nurses and resident physicians). To allocate indirect costs into each procedures, Fully distributed cost and Attributable cost method(FDC-ACM) was applied. Methods  The findings suggest that the current fee schedule for Korean medicine should be revised to reflect the actual resources that physicians invest into clinical activities, in order to avoid a distortion of physicians' behavior.  Calculated RBRVS for Procedures of Korean Medicine Results  Included Procedures of Korean Medicine  The resource-based relative value scale (RBRVS) has been increasingly used to determine payment for Western medical services. In Korea, the National Health Insurance recommended that the RBRVS be introduced into traditional Korean medicine as well as conventional medicine. Back ground Conclusion Conclusion  We found that the actual resource-based relative values for Korean medicine services were statistically different from the current values, with a narrower variation in value distribution. #199939 criteriaN% age ~399250.6 40~496133.5 50~2915.9 sex Male17093.4 female126.6 Clinical career (year) ~9~97943.4 10~197239.6 20~29168.8 30~158.2 total182100.0 procedureNHI covered Consultation ․consultation, pattern analysisYes ․classification of Sasang types No ․prescription for crude herbal drugs ․prescription for herbal preparationsYes Examination ․Ryodoraku test Yes ․Pulse detection test ․Meridian function test ․Sasang questionnaire test No ․Iris test Acupuncture, Moxibustion, and cupping ․simple acupuncture treatment Yes ․acupuncture on back and spine ․acupuncture on knee ․electrical stimulation for acu-needle ․acupuncture on abdomen ․penetrating acupuncture ․acupuncture into nasal cavity ․LASER acupuncture ․Ear acupuncture ․Saam acupuncture No ․Herbal acupuncture ․direct moxibustion Yes ․indirect moxibustion rehabilitation ․cuppingYes ․Chuna therapy No ․Aroma therapy ․Dietetic treatment ․Bloodletting  Characteristics of Respondents PWPERBRVS consultation 1st visit 197.75222.57210.86 Pattern analysis - 1st visit 165.885.38123.31 consultation - repeated visit 149.9799.94123.54 Pattern analysis - repeated visit 126.0682.28102.93 Sasang diagnosis 223.6773.89144.54 prescription for crude herbal drugs 181.99108.92143.39 prescription for herbal preparations 105.95149.27128.84 Ryodoraku test 147.51141.88144.53 Pulse detection test 194.97170.46182.03 Meridian function test 245.43230.87237.74 Sasang questionnaire test 198.33110.76152.07 Iris test 186.3299.24140.32 simple acupuncture TX.
