3 What is a “Near Miss”? A situation or behavior where injury or property damage could have occurred but did not.

4 “Near Miss” situations include:
Events where a safety system failed to perform as it was designed to, (e.g. a fire pump malfunctioned).

5 And “Near Miss” situations are:
Events where environmental damage could result.

6 Frank Bird’s Near Miss Model
3000 Unsafe Acts & Conditions Accident 1 Near Misses 600 For every And approximately To describe the importance of this practice we will briefly review the loss pyramid model developed by Frank Bird. Frank Bird study indicated that for every accident, there were 600 Near Misses and 3000 Unsafe Acts and Conditions . CLICK If you had a single accident in your facility. 7 Unsafe Act We said, unsafe acts and conditions cause near-misses and accidents. So let’s illustrate the unsafe situations that prepares a near-miss. CLICK Here is a driver doing an unsafe act. Not stopping at a stop sign is an unsafe act . Repeated unsafe acts results in unsafe habits. They think they will get into trouble They don’t want to ruin the safety record and lose out on their safety awards They are concern about their reputation as a ‘whistle blower’ (someone who tells on others) They don’t want to be blamed for stopping work They don’t understand how important it is to report incidents

37 In Summary Because there is a “no blame” policy employees can share their near miss experiences instead of covering them up or ignoring them Aramco makes it easy to report near miss events either verbally, written or electronically Reporting makes it possible to put controls in place to change unsafe situations or behavior

38 Remember All 'Near Misses' are Accidents waiting to happen, they could happen to you or one of your co-workers! If we regularly report Near Misses, then it is more likely that we will avoid Accidents. If the Problem is not reported, it will not go away!
