C&P Care Homes by numbers 200 Care Home (≈ 6100 beds ) 135 Homes provide care for 65+ (≈ 5400 beds) 56 Nursing homes Forecasted non-elective admissions from CH ≈ 3466 (17% increase compared to last year) ≈ 3500 ambulance conveyances from CH Health cost of £11 million for acute admissions

C&P Care Home Group Overview Multiagency membership Countywide coordinated approach to: – Address issues – Promote developments – Pilot new initiatives – Gather intelligence & data Aims – Improving the Quality of Care delivered by Nursing and Care Homes – Improving the integration of Care homes with the local health system – Monitoring quality at strategic and system level – Provide support / intervene where appropriate – Reducing attendances and admissions to/at hospitals from care homes – Develop an effective single CCG wide discharge protocol for Care Homes

Care Home Support Service – Care Home Educators The combined elements of the Care Home Support Service will ensure resource is coordinated and targeted appropriately to deliver the outcomes of the CHG. Four types of intervention: Strategic Education and Training Case Management Leadership
