13 CLINICALLY INTEGRATED NETWORK Baxter Regional Medical Center – Northern Arkansas (Retirement Community) Jefferson Regional Medical Center – Southeast Arkansas (AR Delta) Outreach being conducted from hospital to get LTC facilities connected to SHARE Identifying referral partners to get them connected to SHARE benefit from: o Assistance in meeting their goals for the Meaningful Use Transition of Care; o Reduced readmissions by providing proactive, patient centered post-discharge; o Connectivity with affiliated or non-owned referral practices.

16 SHARE LTPAC Survey Survey was sent to LTPAC facilities Results demonstrated the following:  Most LTPACs are still paper based  Two vendors have market share for HIT in LTC  Most use Practice Management System/lite HER  Most do not have to effectively communicate securely

17 Many LTPAC facilities that have EHR are still using a hybrid model with paper/light EHR. LTPACs need to be able to communicate securely for care coordination as well as improve Five-Star ratings. 3 WAYS TO SHARE CHALLENGES FOR LTPAC

18 SHARE CAN HELP SHARE’s SM provides the ability to communicate securely for TOC while SHARE’s VHR gives the full patient record from all participating providers SHARE has received grant funds from the Department of Health and Human Services’ (HHS) Office of the National Coordinator for Health Information Technology (ONC) to offset LTPAC connections. The funding is being used to expand the adoption of health information exchange technology to long-term and post-acute care providers, behavioral health and Eligible Professionals.

21 SHARE SUCCESS STORIES Cave City Nursing Home is thrilled to be using SHARE's VHR, a web-based portal which provides access to the medical record with just an internet connection! I access the Virtual Health Record to obtain up to the minute lab reports from White River Health System. No more waiting for later in the day faxed results when our physicians need the most current lab values. Our physicians request the lab value and we show current ones. I can easily pull our patient's lab history in the VHR and record it with the current results. In the past I had to pull every chart that was needed for comparison.
