23 Transition Planning for SCSEP Participants If the Clock Has Run Out… Make referrals for employment or other assistance Ensure the participant has a budget to follow and is signed up for appropriate social services (ASAP!) Get permission and alert the participant’s support network (if any) to their increased vulnerability

29 Transition Planning for SCSEP Participants Plotting Out the Exit Process Triage participants based on tenure – first in, first out Chart out participants targeted for exit in each quarter –Allow appropriate time to inform and prepare each group for exit –Continue process with new group exiting each quarter
