2 www.somcomhealth.nhs.uk Providing World Class Local Community Services BACKGROUND The Call for Action – Health Visitor Implementation Plan 2011 – 2015 was launched in February 2011 The overall purpose of this sets out to expand and strengthen the Health Visiting Services Incorporates four main themes: Workforce expansion Healthy Child Programme Building Community Capacity Family Nurse Partnership

3 www.somcomhealth.nhs.uk Providing World Class Local Community Services WORKFORCE EXPANSION The Government has committed to an extra 4200 health visitors by 2015. 411 across the South West by 2015, thats over 100 per year on top of what already exists Somerset is ranked 4 th out of 13 in South West. ( based on infant mortality rates) Therefore we can expect to have: 13 – 2011/12 13 – 2012/13 12 – 2013/14

4 www.somcomhealth.nhs.uk Providing World Class Local Community Services RECRUITING AND RETAINING ●Universities have been asked to provide flexible approaches to the delivery of Health Visitor training, to include different and variable access routes ●Cornwall have developed a Band 5 + practitioner which is accredited with Plymouth University and is currently being considered for registration on Part 3 of the NMC Register ●Practice Teachers are being prepared to provide new models of supervision, assessment and mentoring to support this number of students

5 www.somcomhealth.nhs.uk Providing World Class Local Community Services HEALTHY CHILD PROGRAMME ●Somerset will build on previous work such as the Pilot in Sydenham Bridgwater and the further pilot across the County and the development of increased number of skill mix staff ●Data collection systems have been developed to capture the number of contacts and nature of contacts within the programme ●Will be required to embed the Healthy Child Programme in full

6 www.somcomhealth.nhs.uk Providing World Class Local Community Services FAMILY OFFER – THE NEW SERVICE WHAT IT MEANS FOR FAMILIES Universal Contacts: will be offered to all families Antenatal – after 28 weeks and before 36 completed weeks of pregnancy New Birth – between 10 – 14 days 6 – 12 weeks – include assessment of maternal mental health 6 – 12 months – include systematic assessment of development 2 – 2.5 years development review 3 – 3.5 years - working with Early Years Providers to complete a development review

7 www.somcomhealth.nhs.uk Providing World Class Local Community Services UNIVERSAL PLUS- FAMILIES WHO NEED RAPID RESPONSE AND SPECIFIC INTERVENTION ●Breastfeeding ●6 – 8 weeks postnatal contact ●Delivery of The CONI programme ●Sleep management ●Weaning management ●Parenting advice A personalised care plan is developed as a result of a Family Health Needs Assessment, this is agreed with the family/client and expected outcomes agreed and monitored

8 www.somcomhealth.nhs.uk Providing World Class Local Community Services UNIVERSAL PARTNERSHIP P- LONG TERM SUPPORT Working together with others to provide services to families who have more complex and enduring issues Safeguarding and Child Protection Children with additional medical needs Disabled children Teenage mothers Adults with mental health or substance misuse

9 www.somcomhealth.nhs.uk Providing World Class Local Community Services BUILDING COMMUNITY CAPACITY ●This part of the programme requires Health Visitors to: ●Enhance skills and strengths of communities helping them to address their own needs ●20 Early Implementer sites across the country – will report on how this can be implemented ●University accredited course, developed by Northumbria University ●Uses Blended Learning – take a project in community over a 4 month – 1 year ●Only available to Early Implementer sites currently but will be rolled out

10 www.somcomhealth.nhs.uk Providing World Class Local Community Services HEALTH PROMOTION – CORE PROGRAMME ●Accident Prevention – Child Accident Prevention Strategy ●Emotional Health and Wellbeing – attachment/bonding – new research into neuroscience and brain development ●Prevention of Obesity – Healthy Eating ●Breastfeeding – Improving rates and sustaining breastfeeding up to 2 yrs Delivered within Children centres, in many areas this is already happening - making best use of resources and skills in each area.

11 www.somcomhealth.nhs.uk Providing World Class Local Community Services FAMILY NURSE PARTNERSHIP APPROACH ●The programme requires the expansion of the Family Nurse Partnership ●Somerset Community Health have adopted a training programme to support the development of skills and attributes associated with strengths based approach ●Developing a service specific to young parents ●The service will include a model for supervision and facilitation ●National Programme are expected to report findings in 2013

12 www.somcomhealth.nhs.uk Providing World Class Local Community Services CHILDREN CENTRES Named Health Visitor for each Children Centre – first point of contact Healthy Child Meetings ( or ARCHEY) to discuss cases and agree Lead Professional role Monitor progress of agreed outcomes for each case Information Sharing – Patient and Practitioner Services – New Birth data Case specific – with consent from family/client Joint working in the delivery of groups/ registration on e-start Shared objectives and reporting - acknowledging commitment of both services

13 www.somcomhealth.nhs.uk Providing World Class Local Community Services GENERAL PRACTICE AND MIDWIFERY SERVICES ●Named Health Visitor for each Practice ●Named Health Visitor for each New Birth ●Improve/sustain uptake of immunisations ●Provide information and support regarding newborn screening – blood spot results, newborn hearing screening ●Provide information for Health reviews and assessments ●Signpost to other services, smoking cessation, family planning

14 www.somcomhealth.nhs.uk Providing World Class Local Community Services ACTIONS REQUIRED Strategic Action Plan - Steering Group developed - Ethna Bashford, Lead Officer Children and Young People and Commissioning Lead Julie Yates Operational Action Plan - Working group developed – Bernice Cooke, Deputy Head of Public Health Nursing Task and Finish groups to manage the implementation of the work streams over the next 3 years
