Main Menu Home Display a summary of the status of all of your surveys. Select a survey to work on by clicking on its name. Change the status of a survey using the drop-down menu. Note: This is the only place from which you can close or delete a survey. Create new survey Enter the header (title, logo) and contact information for a new survey. Distribution List /Add Respondent Add a new email address to the pool of potential respondents. Distribution List / Create Target Groups Create or modify a distribution list from your pool of respondents. About ePhocus View a description of the ePhocus tool.

Edit Manage Survey Information View, and modify the header (title, logo) and contact information for an existing survey. Manage Purpose and Instructions Enter or change text describing the purpose of the survey, instructions for completing it, and a consent, if necessary. Create/Edit Questions Enter or change question text Enter or change answer options. Re-order or delete questions. Enter or change skip patterns.

Check View Survey View the survey as it will appear to respondents. Print Survey Generate a hard copy of the survey identical to the online version.

Make Available Publish Freeze the design of the survey and make it available for data collection on the web. Distribute/Add New Respondent Add a new email address to the pool of potential respondents. Distribute/Create Distribution List Create or modify a distribution list from your pool of respondents. Distribute/Assign Distribution List Associate the survey with a distribution list. Distribute/Send Survey Add other email addresses, not in the distribution list, to the list of people who will receive notification of the survey. Freeze the design of the survey and send email to the selected respondents, giving them a link to the survey. Kiosk Freeze the design of the survey and make it available for data collection in a kiosk environment. Kiosk causes survey to loop upon submission.

Analyze Analyze Results Generate bar graphs or pie charts for individual survey questions. Export Results Generate an Excel spreadsheet containing the data from a survey.

Data Checking Data Type Checks (numeric yes/no) DATATYPE information does exist for Permissible Values Is it recorded consistently? Range Checks (1-7, 99) HIGH VALUE, LOW VALUE for Permissible Values, but what about 99? Formatting (99/99/9999) Formatting information exists under Value Domain Details (Display Format, Max Length, Min Length, Decimal Place), but how should it be implemented?
