5 Download the New Dual Credit Course Code Request form Make sure you are using the correct form: Request for Dual Credit Course Code(s) 2016-2017 (for College-delivered dual credit courses) Do not use this form for team-taught dual credits For team-taught dual credits, use the Ontario curriculum course code and “flag” team-taught in your SMS system Be sure to complete the section below: Date: College: Program Coordinator: Note the submission deadlines: 5 SemesterDates Semester I – all new coursesJune 17, 2016 Semester II – all new coursesSeptember 16, 2016 Semester II – additional coursesNovember 21, 2016 May/June or summer programs 2017March 10, 2017 Additional new courses for summer 2017April 28, 2017

6 To Request a New Code For NEW course code requests, please complete the table below. All fields are mandatory except for OSSD Credit Value. College Course Name College Course Code College Course Description Language of Instruction E/F Hours of Instruction Passing Mark (e.g. 55) Proposed Ministry Course Code OSSD Credit Value Determined by Ministry The college course name and college course code should be exactly as they appear in the college course calendar. These two items will appear on student’s OSTs exactly as they appear here. We ask for the college course description …but it does not appear on the “official” list of dual credit course codes that is distributed to SMS vendors, board OnSIS leads and RPTs. The description is helpful to us if we have to create a course code for you. 6

7 To Request a New Code Language of instruction: select “E” or “F”. Hours of Instruction: for a full credit, this must be at least 42 hours Passing Mark: this will be picked up by the SMS system at the school level. In order for students to earn any credit, their mark must be equal to or greater than this. For example, if the passing grade is 70% and your student earns 64%, the system will prevent a credit from being awarded. 9 To Request a New Code If you select the.xls file, you can sort the list by code or by college. Check to make sure that you are not currently using the code you want for a different course at your college. Check the “Existing course codes by subject” If nothing seems to fit, indicate the first 1 or 2 characters if possible Existing Course Titles by Subjects and Codes for 2015-2016 (updated Sept. 14/15 ) For college-delivered dual credits, choose an appropriate code from this list of existing codes. If you can't find an appropriate code, then identify the subject discipline and area of specialization from the list below on your course code request form -- and a code will be assigned by the Ministry of Education. Ensure you have not selected a code already used for a course at your college. See the Dual Credit Course Code List (http://www.scwi.ca/coursecodes.php) See the tab in this Excel document, "EDU Course Codes FYI" for more information on dual credit course codes. 15 Updating an existing code For updates/changes to existing codes, enter the current code in the Proposed Ministry Course Code column and insert any other changes in the appropriate column(s). Remember that once you change any of the details for an existing code, the previous information will no longer appear on student transcripts. 16 Next steps… Submit the completed form to your RPT chair for forwarding. A revised course code list is usually sent out within two weeks of the submission deadline. Check your codes to make sure all requests have been included and to confirm the codes that have been assigned. If there are errors or omissions, contact Phil.Hedges@ontario.ca as soon as possible. Phil.Hedges@ontario.ca The final list is then sent to SMS vendors, to district school board OnSIS leads, to OCAS and to OUAC. It usually takes a few days before you can use the codes at the school level. 16
