Enter your NCID and password Remember that NCID passwords expire every 90 days and you MUST keep yours updated or you could be locked out of the system. You should receive email reminders if it is almost time for your password to expire.

Go to “school menu” and click on Implementation Inventory.

Select your school from the dropdown menu Click on “Create a new Implementation Inventory Record”

Enter the person’s name filling out the form Click on “Create Implementation Inventory Record”

Make sure the majority of your PBIS Team is present Read and discuss the Fidelity Statement and have members sign that they understand Complete the survey 2=in place, 1=partially in place, 0=not in place The score is based on the majority vote of the team

Answer all of the questions regardless of whether or not your team has had all three modules of PBIS Training You do not have to click on the “Apply Changes” button after every section When you are finished, click on “Apply Changes and View Updated Totals” at the bottom of the screen

Scroll to the bottom of the screen to see how your team scored If you are close to 80% but not quite on the Core, Supplemental, or Intensive Practices totals, you may want to revisit some of the questions in those sections to ensure you are not underscoring yourselves as a team NOTE-Teams that have not had Modules II or III or just completed training may not have scores at 80% on the Supplemental and/or Intensive sections and there is nothing wrong with that

If your team is satisfied with the scores, scroll to the very bottom of the screen Click the “Yes” circle after the ready for final submit question Click SAVE at the end of this question Once you click save, you may not go back to change scores
