4.  Use Stage 1 requirements to train your providers and staff  Can be found in IO Practiceware’s MU Memo  Pick a reporting period  A reporting period is 90 consecutive days that ends by September 30, 2014  If you choose a reporting period between September 30 th and December 31 st, you will be subject to a payment adjustment in 2015 If Participating for the First Time in 2014

5.  Submit your Attestation before October 1, 2014  Attesting after October 1 st will allow you to receive incentive payments AND be exempt from penalty adjustments in 2016  BUT will subject you to penalty adjustments in 2015  During your attestation  Be Sure to Record your CQM aggregate numerator and denominator. **You have the ability to earn $12,000 in 2014, $8,000 in 2015, and $4,000 in 2015 (up to $24,000 total)** ** For not participating, you will be penalized 1% each year. You will received 99% of the Medicare Physician Fee Schedule Covered Amount in 2015, 98% in 2016, and 97% in 2017 and beyond** If Participating for the First Time in 2014

7.  Based on your Results, decide on a reporting Period  April 1 – June 30: You must have IO version 8 installed before April 1  July 1 – September 30: You must have IO version 8 installed before July 1  October 1 – December 31: You must have IO version 8 installed before October 1  Submit your Attestation  No later than 2/28/15  Submit your CQM data electronically  Failure to do so will result in penalty adjustments in 2016 If you are Beyond your First Year in 2014

8.  A requirement of MU is submitting CQM to CMS electronically  You will be submitting CQM data via IO using a registry such as AAO’s IRIS Registry  Website: http://www.aao.org/iris-registry/http://www.aao.org/iris-registry/ Clinical Quality Measures (CQM)

9.  You only need to report CQM for the same reporting period as MU  However, reporting for the full year will also satisfy PQRS  You will find what measures are available and how to report on the IO Practiceware Support Webpage Clinical Quality Measures (CQM)

10.  If you submit electronically, you will satisfy your CQMs as well for the entire year  If you ARE NOT participating in MU and sending CQM data for the whole year…  You may use the alternative methods for satisfying PQRS found on the IO Practiceware Support Website Physician Quality Reporting System (PQRS)
