2 Clinic Services  Services are available in Park City, North Summit, and South Summit.  Services include, but are not limited to, Inpatient and Residential Care, Outpatient Mental Health and Substance Use Treatment and Prevention.  Services are provided in the following settings: o Office o Community/Home o Schools o Other Locations

4 Staff  Clinicians/Direct Care Providers o Full Time Licensed Mental Health Therapist’s - 5 (2 Bilingual) o Full Time provisional license - 1 o Part Time LMHT - 1 o School Based Independent Contractors (LMHT’s) - 5 o Full Time Case Managers - 4 (1 Bilingual) o Part Time Family Resource Facilitator - 1 Bilingual o Part Time Respite - 2 o Peer Support Specialist - 1  Medical Providers o Board Certified Adult and Child Psychiatrist - 1 (8 hours/ week) o Full Time APRN - 1 o RN - 1 (30 hours/week)

7 Highlights of 2015/2016  Implementation of School Based Mental Health Services (SBMH) in 2015.  SBMH Services currently being provided in the following schools: o Park City Schools: PCHS, TMJH, Ecker, WSD, WSS (school based and ongoing crisis services) o North Summit (Elementary, Middle, High Schools) o South Summit (Elementary, Middle, High Schools)  Number of youth served: o 2015-82 o 2016-98

8 Highlights of 2015/2016  Implementation of Justice Reinvestment Initiative (JRI) in 2015. o On March 31, 2015, Utah Gov. Gary R. Herbert signed House Bill 348S01 "Criminal Justice Programs and Amendments" into law. o With the implementation of JRI, the State of Utah will reduce incarceration and recidivism rates. o “The criminal justice system as a whole is expected to move toward a system that incarcerates people who pose a risk or a threat to the community and treats those who struggle with addictions but otherwise pose little or no danger to our neighborhoods.” Utah Department of Corrections

9 Highlights of 2015/2016  Summit’s JRI Program is currently providing the following services: o Jail in-reach and group services to incarcerated individuals. o Intensive outpatient services such as case management, therapy and crisis services to high risk/high need individuals who would otherwise risk incarceration.  Number of individuals served in jail: o 2015-67 o 2016-82  Number of individuals served in an outpatient setting in lieu of incarceration: o 2015-167 o 2016-189

10 Highlights of 2015/2016  Creation and Implementation of the Summit Advisory Council in 2015.  Question, Persuade, Refer (QPR) Training (National Suicide Prevention Training) provided to several schools in Park City, North Summit, South Summit as well as Summit County Health Department and Jail Employees.  Collaboration with Parents Empowered, School Districts, the Summit County Health Department and other community agencies/partners to educate the community about the following: o Reduced underage drinking o Bullying o Marijuana o Smoking cessation o Parenting classes
