Global One Health A new concept to meet future challenges Wim H. M. van der Poel, Willem Takken, The Wageningen University Global One Health Core Group May 2016

“One Health” “One Health is the collaborative effort of multiple science professions, together with their related disciplines and institutions –working loaclly, nationally and globally – to attain optimal health for people, domestic animals, wildlife, plants, and our environment” (www.onehealthcommission.org )www.onehealthcommission.org 2 Calvin Schwabe 1927-2006 “Modern advocate of One medicine”

‘Global health’  Is widely used:  Study, research, and practice on improving health and achieving equity in health for all people worldwide.  Emphasis on transnational health issues, determinants, and solutions  Involves many disciplines within and beyond the health sciences and promotes interdisciplinary collaboration  Synthesis of population-based prevention with individual- level clinical care  Adapted from Koplan et al. (2009)

Ebola outbreak West Africa 2013-2015

Socio-economic development Impact on disease  Nutrition:- Obesity, Diabetes - Cardio-vascular diseases  Lifestyle- Cancer - Cardio-vascular diseases  Misuse of pharmaceutical drugs in animal production - Anti-microbial resistance - Food security  Pesticides and toxic substances -- Residues in the food chain

Zika virus outbreak 2015  Flavivirus, 1949 Nigeria  Aedes mosquito vectors  Sexual transmission  Microcephaly suspected to be associated with infection

Global One Health “Global one health is the combined effort of multiple disciplines to improve health of humans, animals and plants within sustainable ecosystems by using an integrated systems approach to come to transnational solutions”. The vision of the Wageningen UR Global One Health team is expressed in an opinion paper “Global One Health – a new integrated approach” by Fresco L.O. et al. 2015, published by the Trilateral Commission.

Determinants of a Global One Health and their interactions

Systems approach to health of animals, humans and plants  Diseases arise within a complex molecular, biological, social and economic system.  Systems models have positive and negative feedback mechanisms to parallel real life (dynamic relationships, feedback loops, interactions and dependencies).  For example: cultural and economic relationships affect people’s interactions with animals and ecosystems.

Global One Health projects 2015, 2016  Heterogenity of mycotoxine infection in batches  Successful intervention strategies in Global One Health  Reduction of particulate matter and particle-born (dust) endotoxins in animal production  Blood-feeding midges as models for the role of microbiomes in the transmission of diseases  Global One Health risks of livestock manure application  Sustainable Food Systems for Healthy People  Emerging zoonoses in relation to the changing socio- economic environment 12
