2 PERSON-CENTERED PLANNING  Person-centered planning is a process for learning how a person wants to live and what is important to him or her in everyday life  Person-centered planning assists individuals and their families as they seek to develop ordinary lives and connects people to their communities

3 PERSON-CENTERED PLANNING  Individuals should be supported to enjoy the right to life and the opportunity to have a good life  A good life is one that is led by the voice of the individual

4 PERSON-CENTERED PLANNING  Person-centered planning  Looks to the future and helps the individual plan for positive outcomes  Puts the person’s talents, goals, preferences, needs, and choices in the center of the planning process  Helps people find and use their voices to state what is truly important to and for them

5 PERSON-CENTERED PLANNING  Person-centered planning  Requires really listening to the person and people who know the individual best and translating a vision for a better life into action plans  Enlists the support of family, friends and professionals to follow through on those action plans and journey with the person to a life enriched by community connections and opportunities to contribute and receive support

7 LISTENING  Listening, even when an individual is speaking through their actions, can help you  Learn about their choices  Know their history  Understand their need and use of assistive devices  Know how they communicate  Respect their hopes and dreams

8 COMMUNITY  Relationships with families, friends and people in the community are very important and at the center of planning  You can support community inclusion by  Helping individuals stay connected to family, friends and community members  Supporting individuals in making new connections and friends

9 SELF-DIRECTION  Individuals are in charge of and responsible for their future  Provide support to give an individual what they need to control their life, no matter how they communicate  Like you, individuals have the right to change their life plan at anytime  Individuals have the right to take risks even when others don’t agree

10 RESPONSIBILITY  Responsibility is a shared effort  Support should be provided as needed  Individuals are responsible for their choices, however we are responsible for supporting them to ensure their health and safety are not compromised
