2.  New Form Replacing Subaward Verification Form;  Once our Prime Award or any subsequent modifications is processed (i.e. award/modification synopsis is issued), PI/PD will use this form to request new Subawards (i.e. Subcontracts and Sub-grants) as well as modifications to existing Subawards;  Signed by the PI/PD;  We will not use/accept the Subaward Verification Form effective July 01, 2013.  Please note that SPA will not work on any Subaward related transaction (i.e. new, continuation, no cost extension, etc…), before receiving a completed and signed Subaward Request Form from department/PI/PD.
