Preparation for Launch August Clinic Flow related to Tablets 1.IT testing and usability meeting will be held on July 23 in preparation for usability testing in clinic in August. 2.Volunteers are central to the clinic flow. Training of volunteers on use of tablets will be completed in August. Given volunteers move across all 7 JCC clinics, all volunteers will receive training. 3.Tablets will initially be piloted in Clinics C and G with CNS patients

Preparation for Launch August Clinic Flow related to Tablets 4.Project manager will be embedded in the clinic to assist patients with docking and printing and to assess impact of tablets on clinic flow. 5.Project manager will coach unit clerks on directing patients. Clinic flow analysis showed this requires one additional step by clerk during check in but has been point of resistance in prior ESAS implementation. 6.Project manager will coach health care aides on tablet set up at start of clinic and tablet storage at end of clinic.

One MD/RN team and their patients will be selected to pilot test the new PROM’s tool Patients arriving to clinic will be greeted and guided through the pilot testing iPEHOC steering cttee patient representative will be participating Feedback will be gained from the patients on the new assessment experience Feedback will be gained from the MD/RN team on the process and impact on care Feedback will be provided to entire clinic

Preparation for Launch September Education 1. Patient information sheet will be distributed by MD/RN teams to all CNS patients informing them about PROMs and tablets in October. And arriving 15 mins early for future appointments. 2. Electronic documentation in EHR will go live for symptom assessment and management in October. 3. Clinics C and G CNS and Gyne teams will have new SM resources available to them on intranet and hard copy.

Preparation for Launch September Education During the second week of September, comprehensive education and training of all CNS staff (RNs, MDs, clerks, volunteers, HCAs) will be launched: -ESAS training -PROM training -Scripts for patient activation -Training on patient activation -Awareness of SMGs in weekly case conference -Training on interpretation of PROMs and incorporation into assessment -Training on documentation of assessment and intervention -Promotion of iPODE “Screening for Distress” course for RNs
