Passports must be obtained and copied for turn-in by March meeting
If you already have a Passport, check the expiration date
Passport Photos
Students hold on to their passports while in transit, upon check in at the hotel, chaperones will collect them and put them in a safe.
Students MUST provide a copy by 1/28

Fill Out Form DS-11: Application For A U.S. Passport
*need Social Security number
**Online or by hand
Submit Completed Form DS-11 *In Person. See website for places to do this
Submit Evidence of U.S. Citizenship*birth certificate
Present Identification
Submit a Photocopy of the Identification Document(s) Presented (Step 4)
Pay the Applicable Fee*minors $95 + ($25 fees)= $120
Provide One Passport Photo *Riteaid, CVS, etc

6. Confirmation of Names
You must logon to Eftours.com and confirm the traveler’s name by January meeting.

7. Transfer to Airport on July 25th
Leaving from CB South High School
Each student pays $30-$40 for bus to airport
Parents pick students up at SOUTH upon return
All students will return to the U.S.

8. Forms to Sign
Medical Profile – must be signed and turned in on 1/28/16
Central Bucks School Board Policies #231 regarding Student Travel (read #8 and #12 specifically) and #227 regarding Controlled Substances. These policies must be reviewed and acknowledged with initials by 1/28/16
EF Alcohol Policy form signed by 1/28/16

9. Fundraising
As stated in CBSD policy #231 of the Central Bucks School District, fundraising is the sole responsibility of the student.
