6 Zero Tolerance Policy Get the message out to the parents before the season No one, except the players, can speak to the referee before or after the game about the call of the game No yelling, criticism, harassment, sarcasm, intimidation or feedback Violators can be ejected from the field Coaches and teams are subject to disciplinary action by SYSA and the BAYS Sportsmanship Committee Youth Soccer League (BAYS). Schedule Fall 2016 ULittles (Kindergarten -2nd Grade) – Season starts - Sept 10th and 11th – No games Columbus Day weekend – Season ends - Nov 12th and 13th 3rd - 8 th Grades – Practices starts - Sept 8th – First games Sept 12th – Games Scheduled for Columbus Day weekend – Season ends - Nov 14th High School – Season starts – Sept 18th – Season ends - Nov 6th

11 Coach Requirements - CORI Forms All Adults must register with MYSA – Go to http://ma- adultinfo.affinitysoccer.com/mbrsite/mbrsite.aspx?siteguid= 92FAC7CB-3EA3-4A52-89D6-1FA0EC6DF027 – IMPORTANT: You must have a digital color headshot photo available from the computer/device being used, to be uploaded as part of the registration process. Editing capabilities are available after the photo is uploaded (cropping, resizing and rotating). Youth Soccer League (BAYS). MYSA Registration

12 Coach Requirements - CORI Forms Youth Soccer League (BAYS). MYSA Registration (Con’t) – Upon entering the Adult Registration Portal, there will be two options available. – RETURNING USERS (blue “Register Now” button). For anyone who has a CORI registered with Mass Youth Soccer within the past two years (July 15, 2014), basic data has already been imported into the system and an inactive account has been created. – Enter the email address used when you completed your most recent MYSA CORI and select ‘Forgot Username or Password’ to retrieve access to account (if an account cannot be found for that email address, you’ll need to register as a new user.)

If registering with Mass Youth Soccer for the first time, or it has been more than two years (July 15, 2014) since your last MYSA CORI registration you must create a new account.

14 Coach Requirements - CORI Forms Youth Soccer League (BAYS). CORI Background Check Online CORI Registration After completing the Adult Registration form, you will receive a confirmation email. Returning Users are done at this point New Users should click the link in the email to go to the CORI Registration Form (which looks like the Adult Registration Form except that the registration type is CORI Registration) Enter Username (email) and Password from above After logging in, input requested information You will need a driver’s license or other government issued ID available Please make sure to enter the last six digits of your social security number, not just four digits After completing the CORI Registration, print out the Mandatory CORI Submission Form using the Print Form button

CORI Background Check Paper CORI Submission Process After printing the CORI Submission Form, you must then present the original CORI Form (not a copy) in person to the SYSA CORI Submitter You must also provide your license or other government issued ID (with photo) Both you and the Submitter must sign the form in blue or black ink. The CORI Submitter will then keep the original CORI form on file

CORI Background Check Questions If there are any issues with this process, please contact the MYSA service provider directly Affinity Sports will have a team of support staff available from 10 am to 10 pm EST Monday – Friday and from 10 am to 7:30 pm EST Saturday and Sunday Also available 24 hours a day, 7 days a week, 365 days a year is their help request ticketing system Call 1‐800‐808‐7195 for help
