3. Log into your Google Account: http://www.google.com

4. Click on the array and select Drive.

5. Click on Create and select Form.

6. Type in a title for the Parent Contact Info form and choose a theme.

7. Type a question to include on the Parent Contact Info Form.

9. Check the box if the question must be answered.

11. Decide choices for the confirmation page.
