1 University-Sponsored Insurance Plan through AIG

2 Who is subject to the mandatory insurance requirement? As a condition of enrollment, all international students, graduate students taking nine or more credit hours, and graduate students with assistantships are billed through their tuition account for medical insurance coverage EACH fall and spring semester that they’re enrolled.

3 Graduate Assistant Subsidy Graduate assistant’s receive a subsidy each semester to offset some of the cost of the insurance premium.

4 Waivers If you do not want to accept the University-mandated insurance policy, you may waive out if you meet the following conditions: – Have adequate and acceptable health coverage currently, and maintain it during academic year Adequate coverage benefits explained: https://www.sa.sc.edu/shs/billing/insurance – Must be submitted by Fall: September 14, 2016 Spring: February 5, 2017

5 Waiver Process Go to: www.studentinsurance.comwww.studentinsurance.com Need USC, and VIP ID to waive Enter your current insurance information Confirmation of pending waiver Email stating approval of waiver **Waiver must be accepted in order for charge to be removed**

6 Coverage Dates Fall – August 1, 2016 - January 5, 2017 Spring – January 6, 2017 – July 31, 2017

7 Key Benefits (based off of 2014-2015) Office Visits – No charge if at Thomson Student Health Center – 80% paid after a $25 copay per in-network outpatient visit – 70% for reasonable & customary (r&c) after $40 copay Rx – At Student Health Services: $10 copay then 80% – All other: 80% r&c after $100 deductible – $20 for most generic brands

8 Key Benefits (cont.) ER – $450 copay per visit (co-pay waived if admitted) Urgent Care – $75 copay per visit; 80% allowable in-network charge; 70% r&c out-of-network

9 Key Benefits (cont.) Dental – Preventive: 100% r&c for oral exam (two per year), cleaning (one per six months) and bitewing x-ray (one per year) – Basic services: 80% r&c for surface amalgam fillings – Other: Injury- 80% r&c up to a max $1,000/year

10 Key Benefits (cont.) Vision – 100% r&c after $20 copay per visit – Max of $300 per year Deductibles – $750 in-network, $1,500 out-of-network Birth Control – Paid at 100% with no copay at SHS Out of Pocket Maximum – $6,350 in-network/$15,000 out-of-network

11 Other Information Total cost: approximate $2,547 Preventive services includes – Depression and STD screenings, tobacco cessation, adult immunizations, routine women’s exams Visit Student Health Services first and be referred to outside facilities to reduce costs Can enroll spouse and children directly with AIG

12 Student Health Fee* $178 per semester for full-time students; $119 for part-time students Includes: General Medicine Center & Women’s Care office visits (Sports Medicine, Physical Therapy and Psychiatric Services subject to an additional fee) Up to 10 individual visits at the Counseling Center per academic year Unlimited group counseling Support groups and workshops Preventive screenings Exercise and fitness consultations Nutritional consultations with a registered dietitian nutritionist Prevention and advocacy services *The student health fee is not insurance; it is mandatory.

13 Not covered in student health fee Radiology, laboratory services and EKGs Prescriptions and over-the-counter medication Physical therapy (may require copayment/additional fee) Physical exams Psychiatric services (may require copayment/additional fee) Individual visits at the Counseling Center over the initial 10 Metabolic rate testing Flu shots, allergy shots and other vaccines Medical supplies used during the administration of services (bandages, sutures, etc.) Sports medicine services (may require copayment/additional fee) Travel consults Minor surgical procedures and other procedures (IV therapy, breathing treatment, injection of medications, etc.) Services received at any other hospital or clinic Emergency transport to any other hospital or clinic
