2. Course Verification Form Distribution Directions 2016-2017 1. Pass out verification forms. 2. Tell students, “Student must sign the form. A parent must sign the form. You must list two alternate choices at the bottom of the form.” 3. Read the directions on the sheet aloud to the students. 4. Pass out the AP/Honors request form to any students needing one. There will be several copies of this in your homeroom envelope. 5. Tell the students that they will not be placed in the AP/Honors class if the form is not attached to the verification form. 6. Tell students the forms are due back by Wednesday, March 30, to their English teacher. (English teachers, I will send you an email regarding this.) 7. Please stress to the students the importance of turning these in on time. If students fail to turn in their form, they may not get in the classes they requested.
