Birth Certificate OR other reliable proof of identity and age, including the student's baptismal certificate, an application for a social security number, or original school registration records. Immunization Records OR proof of immunization exemption. PUHSD school nurses can provide FREE immunizations if needed. Documents that are helpful, but are not required are: Current Transcript, State Test Scores, Transferring Grades and Withdrawal Form, Special Education Records (if applicable), and Guardianship Papers (if applicable) If you have questions about enrollment, please call the school registration office.
