5 Housing Move on Team The Housing Move on Team are part of Herefordshire Midland Heart Funded by Supporting People Working in partnership with Herefordshire Council

6 Supporting People Objectives A programme that delivers quality of life and promotes independence. Services that are of a high quality, strategically planned, cost effective and complement existing care services. The planning and development of services is needs-led. A working partnership of local government, probation, health, voluntary sector organisations, housing associations, support agencies and services users.

7 So What Service do the Housing Move on Team offer? We offer housing advice and support to Vulnerable Adults and Adults with Learning, Physical and Sensory Disabilities, to ensure they are aware of and able to access housing choices. We support individuals develop a housing plan and work with other professionals to ensure all their health, housing and support needs are met.

8 Make informed decision to obtain accommodation that is safe, secure, affordable and suitable for their needs Develop the necessary skills to sustain independent accommodation Effectively manage their finances and maximise their income Develop life skills and support networks Manage all aspects of their health Reduce social isolation through participation in activities including local community activities Engage with other services Achieve more choice and control Customer Empowerment

9 What does the Housing Move on Service do? Inform the customer of housing options Support Customers liaise with other organisation to obtaining Housing Options of their choice Support Customers to ensure all benefits and other income are obtained Support the customer to set up and understand the tenancy/Mortgage conditions

10 What does the Transitional Service do? Support the customer to move into and set up their new home Support the customer to budget, set up direct debits etc Support the customer to become familiar with their new location and local facilities Support Customers to access day services, voluntary, work and education opportunities Support Customers to gain life skills required for independent living Signpost to other professionals

19 Assistive Technology Skype Pilot Assistive Technology (AT) is the name given to technology that helps to support people at home. Ranges from a simple community alarm system, to sophisticated monitoring and sensing devices using wireless and electronic technology. The main objective of any assistive technology service is to support individuals to live independently in their own homes for longer.

20 Assistive Technology Skype Pilot Communication between customer and support worker using video technology (Skype)  3 month pilot (started end of July 10)  Ivy close in Hereford/Starling Grove/Duty Team Office  Interviews with customers, family and staff at key stages throughout project  Staff keeping usage logs

21 Assistive Technology Skype Pilot  Approximately five customers using Skype style technology as an integral part of their individual support plan.  The technology will not replace face to face care contact between customers and support/key workers but will aim to enhance the care package provided to customers  Demonstrate care can be delivered at a cheaper cost
