On admission ask patient if they have a NC Advance Directive. If yes: 1) place copy in chart or ask family to bring in 2) check e-Browser for scanned version from a previous admission – verify that the information is still accurate If no:1) Provide literature if desired 2) Determine if patient wishes to complete 3) If patient wants to complete, notify PVR 4) Patient Advocates cannot explain and notarize the document – Social Work or Pastoral Services can assist Check out advance care planning videos – dukehealth.org (search for “advance directives”) Advance Directives
