VQHSE-MS Objective To provide guideline to contractors to meet the objectives of CNOOC SES Ltd (“CNOOC”) Health, Safety and Environmental Management System and the relevant policies. To ensure that contractors understand and implement CNOOC’ HSE policy, procedures, guidelines, standards and system.

ADMINISTRATION PHASE Risk Assessment:
Risk Assessment, a step to classify the contracted service into the following risk categories: Low (L), Medium (M) or High (H). The risk category shall then determines the necessity of the subsequent VHSE steps Risk Assessment shall carry out by User Department prior to performing pre-qualification The evaluation result from the risk assessment must be approved from User Department Manager, and copies of the risk assessment result must be submitted to QHSE Department and Procurement Department. Prequalification performed for medium and high risk only

ADMINISTRATION PHASE Pre-Qualification process:
Containing HSE topic questionnaires (VQHSE-MS) that they are required to fill and submit to CNOOC HSE Group Contractors who obtain a pass the pre qualification shall automatically entered into the CNOOC Database with one-year validity Contractors who do not pass the pre qualification after the 2nd attempt shall not be participated in any bidding process for a of one (1) year period

ADMINISTRATION PHASE Selection, a step to review the Contractors with coordination to Procurement Department shall choose the best candidate(s) based on technical evaluation (HSE view, technical view and commercial view). Field inspection and auditing could be conducted to ensure the compliance between bid document and the reality in the field.

FIELD IMPLEMENTATION PHASE
Pre Job Activity, is the First Step in the Field Implementation phase of VQHSE Management System, after Contract Award. The purpose of Pre Job Activity (PJA) step is to establish the first communication between Contractors and CNOOC before job implementation. Document that shall to carry out by user dept. while starting the job/project : work method (HSE Analysis, Standard Operating Procedure, etc) HSE planning

Process to conduct PJA : Responsibility : USER Dept. PJA meeting in the office (include kick off meeting) covers of : KPI (Key Performance Indicator) Work Plan All Potential Hazards and HSE issues from each work plan, check all equipment, tools readiness, develop emergency response plan and procedures, review the conditional acceptance, if any, and obtained during the Pre-Qualification PJA meeting - Job Site covers of : Contractor Site Orientation and Finalization of all HSE Requirements

FIELD IMPLEMENTATION PHASE
Work In Progress Period is a period where the service is carried out on the job site. Responsibility : User Department shall be conducted during the contract period (or annually if the contract period is more than one year) User Department may ask for assistance of QHSE Team for conducting the WIP Evaluation to develop and giving some advises to contractors to ensure compliance with the contract document, proposed HSE Plan, Company rules, Regulations and Procedure. to verify that the contractor implements the Contractual HSE Obligation and Conditional Acceptance items.

FIELD IMPLEMENTATION PHASE
Final Evaluation is a step to conclude the contractors’ HSE performance during the Pre Job Activity and Work In Progress. This evaluation shall be stored in VQHSE Administrator and shall be used as a consideration when deciding to employ the contractor or not for future projects. Upon completion of the contract, User Department and QHSE Team shall carry out a final evaluation to ascertain how the contractor carried out the work/project. Final Evaluation shall be based on result of Pre-Job Activity and Work In Progress Evaluation Reports.

FIELD IMPLEMENTATION PHASE
The contractor shall present and submit final HSE Report (with evidences) to User Department and QHSE Team, as follows: - KPI - HSE performance - HSE issues - Injuries, damage, incident and near misses reports - Training conducted. - Follow-up of WIP Evaluation results The results from Final Evaluation shall be communicated to contractor. The final evaluation document shall be submitted to QHSE department to be filed in the VQHSE database. The recommendation based on the final evaluation document is used as reference for the next/other bidding program.
