Recognition  Chapter On line application can be used for state/chapter awards, independent of national recognition.  National Chapter applications can be used and forwarded for National Recognition. Chapter chair must submit. National Office receives the form and verifies there is an online form for the nominee and that dues are paid by each nominee. The office then forwards the completed recognition forms to chapters in the appropriate region for balloting and selection of national /regional recognition recipients.
