Account information is usually preprinted in this area. Check to see if it is correct. Record the names of all persons to be collected for this case. Fill in if the account provides/requires this information. Instructions for the Client Authorization Form All shaded areas must be completed. All other information may be provided if the person collected provides the information or the information is requested by the Account. If possible, photocopying the ID card is also helpful. Some direction is provided in the boxes below. Record the amount of money collected or “N/A” if none. Note: only the Account can provide additional addresses – not the persons being collected. Indicate the race of the individual collected. If other is used indicate what “other” means. Inquire as to Transfusion and Bone Marrow (Stem Cell) Transplant status on all collected persons.

Jane Doe Baby Doe 5/28/14 Instructions for the Chain of Custody Form All shaded areas and thumbprints must be completed. The information is requested by accreditation/certification organizations. Some specific directions are provided in the boxes below. Take an instant photograph of the persons collected and attach. If a camera is not available, photocopy their government ID card and attach. Have the persons collected date, print and sign their name. Provide the address where the specimens were collected. Fill in this area even if you already completed the section as collector The person photographed should print and date the photo. Collect thumb prints if possible on all collected persons. Print, sign and date as collector. If witness present have them sign and date. Do not write in this area. This is completed when the case reaches the laboratory.

Now we are ready to begin the collection process.

If the Specimen Collector and Person Packaging part of the form is not complete it will hold up the processing of the case. You will also be sent a certificate/affidavit as appropriate, that you will have to fill out and have notarized and sent back to validate that you collected and/or packaged the specimen(s).

Please note the buttons on the side of the camera. Always notice how many pictures you have left in this view screen to prevent running out of film.

Remove battery cover and insert batteries.

Press the power button to turn the camera on.

Open the back of the camera and open the box of film.

Insert film as shown.

Press the button shown as if you are taking a picture to eject the cardboard from the film pack. Remove the cardboard And the camera is ready to use.
