Go to the website at: http://www.iseopms.org/ Use your 7-digit project ID as your User Login. Use the 8-digit password you received via email.http://www.iseopms.org/ If you do not have your password or are unsure what your User Login ID is, please contact your Westat contact person or email AISLhelp@westat.com. There are two ways to move between questions: After you have answered the questions on a page, hit to go to the next page. Use the question guide in the upper left of the webpage to go to another page of questions. Do not use the “Back” button in your browser to go to a previous question. Instead, use the question guide to move to a previous question. Navigating the Survey

Each section with more than one page of questions has a question guide. The guide is color-coded to help you navigate the system. You can click on any available question (light blue, green, or red) to skip to it. When you have answered all the questions in a section, click “Submit.” This will mark the status of the section as “Complete” on the Section Menu page. If you have not answered all the questions in a section, save your current responses by clicking “Save and Continue.” Then select the “Section Menu” link at the top or bottom of the page. Doing so will return you to the Section Menu where you can navigate to any other section.

Baseline Survey: Westat reviewed your project’s AISL proposal and identified information that we could pre-fill into the OPMS to reduce the time you spend entering data. Annual/Closeout Survey: These surveys are automatically pre-filled with information you entered in the previous survey. You may change, delete, or add any information. Your project has the final say over all information about your AISL grant in the OPMS. Navigating the Survey

No. You can complete the report sections in any order. You can also start sections, save them, then exit and return later. You will be able to exit and re-enter the survey and the individual sections as many times as you need to until you submit the report. Once you have submitted the complete report, you will not be able to re-enter the report. Navigating the Survey

Yes. Anyone whom the PI designates and provides with login information can enter information. Additionally, more than one person can log into the survey and enter information simultaneously. However, if more than one person is entering data in the survey at the same time, we recommend that you work on separate sections. Additionally, the PI should be the one who reviews the report and hits the final submit button when all information is complete. Once the report has been submitted, you cannot re-enter it without talking to a Westat contact person. Navigating the Survey

This example comes from the Key Personnel section. 1. Type the name of the individual. 2. Hit the “Submit” button. Navigating the Survey

The submitted name becomes a link. Click on the link to answer questions about the individual. You can delete submitted individuals. After adding each individual, a new text box appears. You can add an unlimited number of individuals. An information status will appear to tell you if information on an individual is complete. Navigating the Survey

Enter venue name. Enter venue zip code. Select type from drop- down list. If “other,” enter type of other. Hit “Submit” button. Navigating the Survey

In some sections (Project & Lead Organization, Key Personnel, Organizational Partners, and Accomplishments/Lessons Learned) A, you can leave some questions on a page blank and return at a later time to complete the section. (Note: Accomplishments/ Lessons Learned is not in the Baseline Survey.) In all the other sections, you must answer all the questions on a page before continuing. If you do not know the response to a question, you can save the work you have done so far and exit the section, then return to it when you have found the answer. If you do not understand a question or do not know how to get the information necessary to answer the question, please contact your Westat contact person or email AISLhelp@westat.com.AISLhelp@westat.com Navigating the Survey

A number of validations are built into the system to ensure that data are as accurate as possible. If you cannot,, please check for any pop-up alert messages or alerts in red text above a question. Common problems are entering letters instead of numbers, not specifying an “other” category, and leaving a required question blank. Navigating the Survey

A section is complete when you have answered all the required questions in that section. There are three ways to know if a section is complete. 1. If you are in a section, the question guide on the left of the screen will be all light blue (and possibly black, if a question was not applicable to your project). 2. When you reach the end of the section and hit “Save and Continue,” you can select the “Submit” button. 3. If you are on the Section Menu, the “status” for that section will be marked “Complete.” Navigating the Survey

Before all sections are complete, the status at the bottom of the Section Menu reads: “Submission Status: Not Complete.” When all sections are complete, the status changes to “Complete” and a “Submit” button appears. Before you hit the “Submit” button, please select “Print” at the top or bottom of the page to print and save PDF copies of your answers and make sure that all the information is correct. Once you have submitted the report, you will not be able to change any responses without first contacting a Westat representative. Navigating the Survey
