1 A free, secure and simple network of front-line workers in your area Connect, communicate, collaborate

2 The challenge Communication across agencies can be difficult and time consuming. When a multi-agency approach to client care is hard to achieve, gaps in a client’s care can form.

3 Patchwork is an online super-charged contact directory that helps frontline practitioners connect and coordinate client care. It is free, secure and easy to use. It reveals hidden networks. What is Patchwork?

4 Patchwork in NSW Patchwork has been successfully used in some FACS districts for over 18 months. There are over 4000 agents using Patchwork.

5 Patchwork in HNE Patchwork roll out began in November 2015. There are over 800 agents signed up to use Patchwork. HNE has 4 coordinators and 1 administrator.

6 Patchwork in HNE Network growth is increasing steadily, however client information will need to be loaded by the registered agent. It is not preloaded. There is some onus on the user to update, invite and assist in the ongoing growth of Patchwork. Activate your account. As at 1/06/16

7 How does Patchwork work? A frontline practitioner registers for Patchwork. A frontline practitioner checks if their client is already registered, or registers them on Patchwork, once consent is given. Other practitioners can join the support team around that client, or be invited to join. Support workers can then quickly and easily access each others’ contact details to coordinate client care.

8 What information does it store? Patchwork stores basic information so clients can be identified (name, DOB, postcode) and practitioners can be contacted (name, agency, work contact details). Patchwork is not a case management system.

9 Practitioners are required to follow their organisation’s privacy, consent and best practice guidelines and policies. Patchwork does not record case management information. What about privacy?

10 How does Patchwork help? Patchwork helps you visual and track your client’s support networks. It saves you time when collaborating across agencies. It can help you build a team of support for a client.
