What this Effort is All About Aim of 25 Cities Effort How? Accelerate the pace and integrate our efforts towards ending Veteran and chronic homelessness in 25 cities with the largest concentration of homeless Veterans 1.Build and strengthen elements of Coordinated Assessment and Housing Placement 2.Strengthen and integrate data systems 3.Integrate VA efforts on homelessness more seamlessly into broader community efforts

Leading Change in Your Community

Role of Leadership Team
7
Complementary Roles Leadership Team Deflects counter- productive forces Puts own credibility and reputation on the line Accepts responsibility for overall Team performance Hard work, may feel thankless Not in it for the glory Community Teams Need to focus energy on designing and implementing a new system

Strategic Choices for your System
10
Your Focus for the First 100 Days Building the Community Based Team Common Assessment Tool Prioritization Clearing the Path Choice Points for a Successful Community Launch
11
Our First 100 Day Project San Diego will focus on building a complete (front and back end) Coordinated Assessment and Housing Placement (CAHP) System in Downtown San Diego. Building the Community Team San Diego will build a Community Team to design and implement a CAHP System during the first 100 Day Project that launches on June 4 th and 5 th. Common Assessment Tool San Diego chose to use the VI-SPDAT for the first 100 Day Pilot and will evaluate performance to determine future use. Prioritization San Diego will build a system focused on Veterans and chronic homeless first. Both individuals and families will be served in the CAHP system. Clearing the Path During the 100 Days, the San Diego Leadership Team will work with the Community Design Team to target resources to the CAHP and break down barriers to it’s use.
12
How Do Communities Make Progress
13
13 1.Use Data to Drive Results: Use data to measure system and program performance and inform resource allocation decisions 2.Leverage Mainstream Resources: Engage mainstream systems and integrate those resources— housing, job training, child care, health care, etc … 3.Be Frugal – Target Wisely: Provide the right intervention at the right time to the right individual or family through a coordinated assessment system Five Key Tactics
14
14 4.Be Smart – Use Evidence: Adopt Housing First practices to offer individuals and families experiencing homelessness immediate access to permanent affordable or supportive housing, without clinical prerequisites or other barriers 5.Expand the Pie Strategically: Use existing resources in smarter ways to help make clear case for new investments of Federal, State, local, and private sector resources to scale the practices and innovations that work Five Key Tactics

Framing Coordinated Assessment and Housing Placement System: Language and Components
16
Why We Need Coordinated Entry
17
Set of common procedures and tools used by partnering organizations within a community To assess and prioritize for most appropriate housing resources based on need Support selection and movement into permanent housing Track overall community performance against goal of ending homelessness Coordinated Assessment and Housing Placement System
18
CAHP System – 6 key components Choosing Common Assessment Tool Coordinating Outreach and Access Points Prioritizing Housing Navigation and Case Conferencing Matching with Choice Coordinated Assessment Housing Placement Performance Management & Communications Platform
19
Building the CAHP System Engine Regular Case Conferencing Housing provider and funding agency engagement in system Housing Navigation Assignment Prioritization Assessment Coordinated outreach Vacancy notification MATCHING AND CLIENT SELECTION! List of dedicated units and resources Eligibility criteria Front End Most vulnerable entered into the system, prioritized and ready for housing Back End Housing supply identified and vacancies entered into the system Housing Document Collection
20
Performance Management & Communications Platform: Data-driven Decisions
21
Using Data in the 25 Cities Initiative Two Connected Platforms will guide our work and track our progress: – Performance Management and Communications Platform (PMCP) – VA 25 Cities Dashboards
22
PMCP – A Detailed Explanation PMCP uses a series of complex algorithms to match a person experiencing homelessness to the most appropriate resource (based on their needs, preferences and eligibility) Example: – =ARRAYFORMULA(IF($FS2:FS<>1, "", IF($CP2:CP="No", "", IF($DY2:DY="Ineligible Immigrant (including Undocumented)", "", IF($CN2:CN=0, "", IF($ER2:ER="Yes", "", IF($EP2:EP="Yes", "", IF($FF2:FF="No", "", IF($FG2:FG="No", "", IF($FN2:FN="No", "", IF($FO2:FO="No", "", IF($FQ2:FQ="No", "", "Shelter + Care")))))))))))) Wait, maybe there’s a better way to explain this…

Visualizing The System (PSH piece only) Common Assessment Tool Aggregate Client Tracker Case Conferencing Tool Match Initiatio n Form PSH Vacancy Form PSH Vacancies Tracker Client Profile
25
Components of PMCP of PMCP Common Assessment (online) Assigns/Triages people experiencing homelessness to the appropriate intervention (based on assessment) Allows providers to coordinate outreach and services Tracks openings/vacancies in PSH, rapid rehousing programs and other programs Matches (via a matching algorithm) people experiencing homelessness to housing and services based on need, preferences and eligibility Allows communities to track progress at the client, program and systems levels in real time
26
Dashboards Overview Dashboards will allow you to track in real time – Number of housing placements – Progress towards ending Veteran/chronic homelessness Consistent with VA’s Gaps Analysis Tool Dashboards are powered by community data

Making Change Happen – The Rapid Results Way
29
How the Process Would Work VisionDesignPlanPilot Scale- up Vision 100-Day project Impact IMPACT Design Build Use Impact
30
Shrinking the Change 100-Day Goal (Result) 2-Year Vision Population focus Part of System Geography
31
Implications for the Way Forward Shrink the change – Population group – Smaller regional jurisdiction Turn marathon into 100-day sprints Empower front-line teams to design it, build it, and use it Set outcome-oriented goals – # people matched and housed – Number of “document-ready” people on prioritized list
