2 Through CoPower and MetLife, you’ll have access to robust, ready- to-go dental plans with underwriting flexibility and relaxed participation, all from a carrier with more than 147 years of industry experience. We offer two MetLife choices: 1.Our SUITE packaged solution 2.Our standalone dental voluntary plans

3 CoPower SUITE bundles MetLife Dental and Life with VSP Vision to create a comprehensive ancillary solution. Available in three customized options:

4 MetLife offers two Dental PPO* options and one Dental HMO (DHMO)** option in this package. Groups with 5 or more eligible employees can select one of the DPPO options or the DHMO option for their employees and their dependents. Groups with 10 or more eligible employees may offer a “dual choice” plan with any two of the three available MetLife dental plans. *Group dental insurance policies featuring the Preferred Dentist Program are underwritten by Metropolitan Life Insurance Company, New York, NY 10166. **Dental HMO plans are available in California through a domestic company named SafeGuard Health Plans, Inc. The company is part of the MetLife family of companies. “Dental HMO” is used to refer to products offered by Specialized Health Care Service Plans in California.

Dental HMO plan benefits are provided by: SafeGuard Health Plans, Inc., a California corporation in CA; SafeGuard Health Plans, Inc., a Florida corporation in FL; SafeGuard Health Plans, Inc., a Texas corporation in TX; and MetLife Health Plans, Inc., a Delaware corporation and Metropolitan Life Insurance Company, a New York corporation in NJ. The Dental HMO/Managed Care companies are part of the MetLife family of companies. "DHMO" is used to refer to product designs that may differ by state of residence of the enrollee, including but not limited to: "Specialized Health Care Service Plans" in California; "Prepaid Limited Health Service Organizations" as described in Chapter 636 of the Florida statutes in Florida; "Single Service Health Maintenance Organizations" in Texas; and "Dental Plan Organizations" as described in the Dental Plan Organization Act in New Jersey.

8 DHMO offers more than 400 covered services. Covered procedures include services such as: Implants Veneers Composite (white) fillings IV sedation General anesthesia Nitrous oxide Orthodontic treatment in progress at initial group enrollment

9 Negotiated fees accepted by participating network dentists generally range from 15–45% below the average charges in a dentist’s community Negotiated Fees refers to the fees that in-network dentists have agreed to accept as payment in full for covered services, subject to any co-payments, deductibles, cost sharing and benefits maximums. Negotiated fees are subject to change.
