3 Patient Management and Registration System (PMRS) A module in the overall MOH/HMIS Web-based Manages all patient records at facility level in 9 Referral Hospitals (20 more planned) Currently used to managed the HEF system in 61 Operational Districts

4 Patient Management and Registration System (PMRS) 123-456-789-X Only number between 0 to 9 10 Digits First 9 digits is series number increase by 1 The last digit is check digit with Universal Product Code (UPC) algorithm to calculate Current Unique Patients Registered in PMRS at RH Level: 256,687

5 IDPoor Poverty Identification System Stand alone database at MOP Soon to be web-based Includes details of every poor family and members Currently used as the basis of HEF eligibility

6 IDPoor Poverty Identification Code PPDDCCVV-XXXX Example: 01020102-0001 Only number between 0 to 9 12 Digits First 8 digit is the gazetteer Code (XX= Province, XX=District, XX=Commune, XX=Village) The last 4 digits is series number start from 0001 to 9999 increase by 1 Geographic CodeHousehold Counter

7 Civil Registration and Vital Statistic (CRVS) Developed for MOP Web-based Records births, marriages, and deaths (including screen for maternal death) Currently used in 10 Communes in K. Cham. Expansion this year to Battambang

8 Civil Registration and Vital Statistic (CRVS) PPDDCC YYMMDD M XXX X Example: 031102 130101 1 585 3 Only number between 0 to 9 12 Digits First 6 digit is the gazetteer Code (XX= Province, XX=District, XX=Commune) The second 6 digits is date of birth The third 1 digit is Sex (M=1, 2=F) The fourth 3 digit is the series number start from 001 increase by 1 The last 1 digit is check digit
