Our Target Populations Age 60+ Individuals with mental and/or physical health illnesses. Individuals with alcohol or substance misuse Living alone Living in rented property Fire Interaction e.g. smoking, cooking, candles Single parents with young children Known to other agencies

What is a Home Safety Check? This a completely FREE service with the aim of reducing fires and improving safety in the home. It is a risk assessment to identify any fire risks or hazards in the home. Burnt cooking and Chip pans Electrical – Plug in Air fresheners, overloaded sockets, mobile phone / e-cigarette chargers Smoking materials / discarded cigarettes Naked flames – Candles, Fires Radiated heat – hot items Unattended appliance – Tumble Driers and Washing Machines It provides personally tailored fire and basic home safety advice which includes making an escape plan and having a bedtime routine. We will also check that any existing smoke detectors are correctly sited and working and where appropriate replace or fit a new one free of charge. Our approach is: A light touch home safety check of all individuals in the home; Identification of risk while in the home; Provision of brief advice; Provision of appropriate risk reduction equipment; Referral to specialist advice and support where appropriate.

Trips, Slips and Falls Interventions Our Aim is to Reduce incidents of Trips, Slips and Falls by: The Introduction of the FRAT and referral to Primary Care/ CRT as appropriate Basic Advice for Occupier on: keeping entrances and exits clear from clutter; Avoiding the need to climb to reach high items Moving with hot foods or liquids Keeping mobile around the home Drinking fluids to avoid dehydration. Providing small PIR LED lighting devices to reduce TSF Basic repairs to carpets and uneven floors or referral to Care and Repair, Age Concern/Age Cymru Currently developing the MECC into the Safe, Well And Independent Living (SWAIL) with MAWW FRS, Welsh Ambulance, Dyfed Powys Police and Hywel Dda University Health Board to enhance the Interventions we Deliver in partnership. SWAIL Project Overview Fire Service Leading the Project Team of Four Caseworkers, one from each organisation MAWWFRS, DPP, WAST and HDUHB plus Project coordinator Distinct area of Carmarthen chosen as a trial Distinct Project Logo, Uniforms and Two SWAIL Vans in place Common Areas to target for people over 60 years old are: Home Fire Safety Checks Reducing Slips Trips and Falls Wider health and safety messages Carbon Monoxide (CO) Healthy Living (including Smoking Cessation) Crime Prevention Scamming awareness Reduce the incidence of repeat victimisation among elderly, vulnerable and disadvantaged people Flood awareness Reduce loneliness and Social Isolation People live in adequate accommodation Home Security Victims of Crime Encourage healthy lifestyles to reduce avoidable 999 and Emergency Department usage ‘Choose Well’ Initiative

Other Holistic Interventions Advice and Referrals to Stop Smoking Wales Increasing Awareness of Carbon Monoxide Health & Wellbeing ie. ‘Choose Well’ Campaign with the NHS Referrals to NEST – Energy Efficiency Measures Basic Crime Prevention Advice and Referral to South Wales / Dyfed Powys Police Advice and Signposting to Natural Resources Wales ‘Floodline’ Referrals to Befriending Services for Social Isolation, volunteering, nail cutting, walking groups, knitting, gardening, art & crafts, music, legal advice, form filling – Age Concern / Cymru Referrals for Scamming – Trading Standards Signposting for Financial Advice – CAB etc Signposting to Western Power Priority Customer Register Safe, Well And Independent Living (SWAIL) and Making Every Contact Count (MECC)
