My Aged Care provides: Information on ageing and aged care services Assistance to navigate the aged care system Service finders to assist people to locate services Fee calculators, to assist in estimating potential costs Referrals to access Commonwealth funded services 3

Assessment Assessment works out how much help, and what type of care and services you may need, and are eligible for. During an assessment you will be asked questions about: Support you are currently receiving Any health concerns How you are managing at home Safety in the home 4

Comprehensive Assessment You will need an ACAT assessment and approval if you have complex care needs and want to: Access aged care services through any level of Home Care Package Receive services through transition care Receive respite care in an aged care home Move into a nursing home. 5

Home Care Packages A Home Care Package provides a co-ordinated package of services tailored to meet specific care needs to: help older people stay in their own homes as they get older, and give them choice and flexibility in the way their care and services are provided. 6

Increasing Choice in Home Care 11 The Australian Government announced significant reforms to home care in the 2015-16 Budget. The reforms will be implemented in two stages. In the first stage, from February 2017, funding for a home care package will follow the consumer. This will allow a consumer to choose a provider that is suited to them and to direct the funding to that provider. The consumer will also be able to change their provider if they wish, including if they move to another area to live. There will be a consistent national approach to prioritising access to home care through the My Aged Care gateway. The second stage will build on these changes by integrating the Home Care Packages Programme and the Commonwealth Home Support Programme into a single care at home programme. This will further simplify the way that services are delivered and funded. The Government intends to introduce the new integrated programme from July 2018.
