Funded by CDC Division of STD Prevention (DSTDP) Primary Goal: To train public health professionals in DSTDP-funded STD programs and PTCs to become QI specialists supporting ASH clinical care within primary care settings How will this goal be achieved? Through extensive training on and practice with QI tools/methods Through in-depth exposure to ASH clinical best practices and their common barriers and facilitators Program Overview

Fellow Requirements Active participation in all instruction: Didactic instruction: on QI methodology and ASH clinical care best practices 11-12 distance-learning webinars 3-day in-person training (in N. Carolina) Self-directed learning: pre-readings and research 1:1 mentoring and coaching: with QI expert trainer Experiential learning: apply tools during final project Peer-to-peer learning: with other fellows

Fellows will lead a Final Clinical QI Project: Preparation Selecting/securing a clinical site partner and project Developing aim statements and goals Collecting data to measure baseline performance Implementation Co-Facilitating a rapid-QI onsite event (“Kaizen”): ~ 4-5 days Follow-up 30-, 60-, and 90-day follow-up meetings with clinic site Supporting continuous QI and providing ongoing TA Collecting data to measure results Report Back Presentation Sharing QI results and lessons learned Information sharing Fellow Requirements

November 2016 - May 2017 ~2 webinars per month 3-day in-person training in March 2017 March - July 2017 Final Clinical QI Projects scheduled August - October 2017 Report-back presentations due Overall Commitment Timeline

Number of Accepted Fellows 1-2 fellows from up to 5 project areas will be accepted (10 fellows in total) Teams of 2 are strongly encouraged to apply Each prospective fellow in the team should complete a separate application but acknowledge the team-based approach

Prospective fellows are required to submit a Supervisor Letter of Support Initialed by the applicant Completed and signed by the applicant’s supervisor Stating that they understand the time commitment and have approved fellow participation in all required activities Supervisor Support

Strong Fellow Applicants Level of QI experience: has experience designing, implementing, and/or evaluating QI interventions Commitment to continuous QI: can articulate the value of learning about QI Organizational support: has the support of their supervisor to participate and to utilize staff time working on a QI final project Position within the organization: participation in the fellowship will increase future QI activities at their organization Clinical partnership: has existing clinical partnerships or ideal targets in mind for Final QI Project

Application Deadline The (1) Fellowship Application [via online survey] and (2) Supervisor Letter of Support [sent via email] must be submitted no later than October 24, 2016. Acceptance decisions will be communicated no later than November 1, 2016.
