10. How do I get reimbursed? You must file a claim. –Which is a request for payment… Complete the claim form and mail it to IEES WITH –the original itemized bill or –Section B completed.

You must still file a claim. –But the health care provider does not need to use U.S. medical diagnosis codes. –The claim does not need to be on an official claim form If the claim is for less than U.S. $100 the Program Coordinator may use a simplified form. –Mail the form to IEES with original bill or receipt from the doctor.
