Welcome Baby Program Overview Home visitation program for pregnant and postpartum women living in and outside Best Start communities and giving birth at participating hospitals Free program providing support and information to pregnant women and new mothers at the hospital and in the home Provide primary health prevention and parent education and introduce families to support services and resources in their community Offered to all families (residing in LA County) regardless of risk factors or socio-economic status

Welcome Baby Program Goals Promote overall health and wellness during pregnancy through first year Increase breastfeeding rates Ensure children have health coverage and receive consistent healthcare Assist new parents in providing a safe and nurturing home environment Link families to community resources

Welcome Baby Program Objectives Increase dental screening during pregnancy Increase screening for maternal depression Increase safety in the home Increase breastfeeding rates: initiation, exclusivity, duration Ensure newborn healthcare coverage Increase immunization rates

Welcome Baby Program Objectives Improve continuity of care: postpartum, well baby visits Increase access to local food resources (WIC) Increase screening for developmental delays Link clients to health and social services Enhance parent-infant attachment

Welcome Baby How the Program Works One-on-one visits in the home, at the hospital and by phone RN, Parent Coach and Hospital Liaison provide personalized support during pregnancy through 9 months Provide support and information on: Nutrition for mom and baby Labor and delivery Home safety before and after birth Breastfeeding, child development Referrals to community resources Program materials to assist with parenting and stimulate baby’s learning Number of visits and materials determined by place of residence and level of need

Welcome Baby Postpartum Hospital Visit Administered by Hospital Liaison Bedside visit following delivery Provide support and information on: Breastfeeding Positive parent-infant interaction Postpartum, newborn follow-up care Complete Universal Risk Screening

Welcome Baby Universal Risk Screening Identifies families level of risk and need Links families to appropriate supportive services Bridges for Newborns Screening Tool Orange County hospitals used for past 10 years Asses families level of risk Determines length of participation in Welcome Baby Program Families with high risk level in need of supportive services receive referral to more intensive home visitation program.
