2 LAB SAFETY CONTRACT Must be signed by you and your parent/guardian or you cannot attend lab!!!

3 LAB SAFETY TEST To pass the test you must score 85% or better You will have to study and retake the test until to reach a score of 85% to attend lab

4 Rules Follow all written and oral instructions OR You may be removed from the activity, receive a failing lab grade, and/or disciplinary action!!!

5 Rules NO horseplay, pranks, or practical jokes

6 Rules Read all directions thoroughly If you don’t understand, reread and then ask for help

7 Rules Leave book bags, pocketbooks, and other items in the classroom. Only bring materials you need to complete the lab

8 Rules No eating, drinking, or chewing gum during activities unless instructed by your teacher!

9 Rules Do not touch any equipment, substance, or other material unless you are instructed to do so by your teacher. Do not perform any activity that is not authorized by your teacher!

10 Rules Keep your work area clean At the end of your lab:  Clean all glassware  Put all materials back where they came from  Wipe down your table with cleaner  Wash your hands

11 Rules Do not leave your assigned work area – especially to talk to people in other lab groups Do not go into storage or preparation areas

12 Rules Wear goggles if you are working with heat, flames, glassware, or chemicals If you obtain contacts or glasses during the term notify your teacher so you can fill out another contract

13 Rules Tie back long hair

14 Rules Wear shoes that cover your feet No flip-flops, ballet flats, flops, or slide-ons

15 Rules Wear lab aprons to protect your skin and clothing when working with chemicals or substances that can stain clothing or cause injury

16 Rules Report all accidents, spills, or injuries to your teacher All items contaminated with bodily fluids must be reported to the teacher

17 Rules Do not touch taste or smell chemicals unless instructed by your teacher If you are allowed to smell – waft the chemical – do not stick your face/nose near the chemical

18 Rules Read label twice and make sure you never remove chemicals or other materials from the lab Do not return used chemicals to the original container. This will contaminate the entire container.

19 Rules Acid spills should be neutralized with a base and base spills should be neutralized with an acid If you spill a chemical on your skin report it to the teacher immediately and rinse with running water in the sink or shower

20 Rules Never handle broken glass Report all chipped or broken glassware to your teacher

21 Rules Do not immerse hot glassware in cold water or cold glassware in hot water It will shatter

22 Rules Handle hot glassware with tongs, clamps, or designated insulating materials

23 Rules Examine electrical equipment for any damages (frayed cords, exposed wires, and loose connections) Make sure your hands are dry before touching electrical equipment Keep electrical equipment away from water

24 Rules Never heat anything unless instructed to do so by your teacher Never place combustible materials near a heat source Never reach over an exposed flame or heating element

25 Rules If something catches on fire use a fire extinguisher Steps: (3 P’s or PASS)  Pull, Point, Press  Pull, Aim, Squeeze, Sweep If someone catches on fire, wrap them in a fire blanket

26 Rules Never leave anything that is visibly heating or reacting unattended Turn off heat sources when not in use All heating items must be placed on designating heating pads

27 Rules Do not point the open end of test tube near yourself or anyone else

28 Rules Always allow ample time for heated objects to cool Hold the back of your hand near the object; if you can feel heat on the back of your hand, the object is too hot to handle

29 Rules Handle all specimens with care Wash hands after handling specimens Do not bring any live or dead specimens unless you receive formal written permission from your teacher

30 Rules Outdoor Activities:  Dress appropriately and protect your skin  Avoid contact with anything unless instructed to do so by your teacher  Do not look directly at the sun

31 Rules End of Activity  Clean your work area  Thoroughly wash glassware and return all equipment to its proper place  Dispose of waste materials as instructed by your teacher  WASH YOUR HANDS AFTER EACH ACTIVITY!!!
