Quarter 1 January 1-March 31 “My Diet is Better Than Yours” Weight Loss Challenge Participation: 16 total teams (4 associates / team) (64 associates) Total Weight Lost: 371.1 lbs 1 st Place Team Lost: 44.8 lbs (25.42%) Prizes – Half way: Subway Lunch / End winner: $50 Gift Cards to Dick’s sporting Goods Lunch And Learn (Topic: Desktop Yoga) Presenter – Roxanne from YMCA Participants: 35 associates Heart Healthy Oatmeal Breakfast *Not for a wellness credit Smoothie Day *Not for a wellness credit Easter Egg Hunt *Not for a wellness credit

Quarter 2 April 1 – June 30 5K & ½ Mile Kids Dash Participation – 59 33 associates 12 family members 14 kids Lunch And Learn (Topic: Seasonal Allergies) Speaker: Dr. Summit Shah and Heather Kern from Premier Allergy Kickoff to Summer Cookout *Not a wellness activity 49% increase from 2015

Quarter 3 July 1 – September 30 Summer Slim Down Weight Loss Challenge Participation – 50 associates Total Weight Lost: 140.4 lbs 1 st place Winner Lost: 18.8 lbs (10.43%) Prizes: $5 Subway gift cards for weekly winners Kohl’s gift cards for 1 st, 2 nd and 3 rd place Lunch And Learn (Topic: Routines & Healthy Lifestyles) Speaker: Steve Van Dyke from Livin Up 2 Fitness Company Picnic Kings Island

Community Challenge During Entire Month of September YMCA Sponsored Event Promotes Healthy Lifestyle and Wellness (and healthy competition) Various Sporting Events Basketball, Golf, Corn hole, Indoor and outdoor volleyball, softball…many more 85 Participants (associates and family) Wellness credit: Associates must participate in 2 activities for max of 1 wellness credit

Ideas for rest of 2016… Step This Way Program o To obtain a wellness credit, you must accrue 225 points over the 8 week period. o OCSC Enhancements (To give some incentive): o For obtaining 225 points, you will receive one raffle ticket o For each additional 225 points, you will receive another raffle ticket. o Example: 225 points = 1 ticket; 450 points = 1 additional ticket; 675 points = 1 additional ticket. o The tickets will go into a raffle for these three prizes. o Wireless Bluetooth sports headphones ($75 value) o $75 Amazon gift card o $75 DSW gift card Sumitomo 4 Miler By donation to help support our United Way Campaign Another Lunch and Learn Sneaker Day United Way Campaign Maintain, don’t gain thru the Holidays

Union County Wellness Consortium Member of Union County Wellness Consortium Supported by the Health Department/ Chamber of Commerce Monthly Meetings Some of 2016 Topics: Union County Health Assessment Review & Strategies Tour of the Ohio Reformatory for Women and understanding their Wellness Programs Routines & Healthy Lifestyles Winter Safety Tips Cancer Prevention and Screening Guidelines 5-4-3-2-1 SEWS volunteers Within county schools Focuses on teaching healthy lifestyles at the 4 th grade level
