How to bring your pet to Guam?
Guam is a rabies free country and therefore requires special care with importing animals, to protect our residents and pets from the risks associated with the spread of rabies. To keep Guam rabies free Guam’s Department of Agriculture (DOA) has issued a quarantine program.
Our following checklist will give you an idea, which steps are necessary to complete the current requirements of importing your pet. Though we are working closely with the DOA this information could vary from the actual requirements and should not be used as a reference.
Please take the time to read the Information Brochure of Guam Department of Agriculture “GUAM Animal Rabies Quarantine Program”, as it gets more into details! In a brief overview, there are 4 different programs:
1) Full 120 Day Quarantine
Your Pet is NOT arriving from an exempt country and has NOT completed or passed the blood test confirming that your pet responded adequately to the rabies immunization and is producing its own rabies antibodies (FAVN) and other requirements.
2) Calculated Quarantine
Your pet has completed all requirements. FAVN titer is at least 0.5 I.U to 0.99 I.U. Your cat/dog will only have to spend the rest of the 120 days (counted from the day blood sample arrived in the laboratory) in commercial quarantine. Allows Pet to live his normal life in the USA or Overseas U.S. Military Installation as much or all of the 120 days while awaiting the move to Guam.
3) 5 Days or Less Home Quarantine
For all pet originating in the mainland USA for 120 days or more AND MILITARY INSTALLATIONS (with proof of documents). FAVN titer is at least 1.0 I.U. After 5 days or less in commercial quarantine your cat/dog could be transferred to Home Quarantine for the remainder of the 120 days. It is required to keep the pet in a strictly enforced Home quarantine.
4) Exempt from Quarantine
Arriving on direct flights from Japan, Hawaii, British Isles, Australia or New Zealand, after staying in the exempted area for at least 120 days prior to arrival, are normally allowed to leave the airport with you, if all requirements are completed and clearance has been giving by Guam Customs and Quarantine at the port of arival.

2. Contact your Veterinarian.
Meet requirements of Guam Department of Agriculture/comply with Brochure STRIKTLY rely on the current Brochure “Guam Animal Rabies Quarantine Program”. In short:
a) Microchip
b) Vaccinations against common infectious deseases
c) Two (2) subsequent rabies vaccination
DO NOT VACCINATE YOUR PET AFTER BLOOD IS DRAWN FOR THE FAVN TEST!!!
3. FAVN TEST (no more than 12 month prior to arrival)
4. Affidavit for Export (Check with DOA current requriments)
For animals origin directly from rabies free area.
5. Send Documents to DOA and follow up
6. Contact Airline and approved Quarantine Facility of choice for your reservation.

8. The 10-Days Health Certificate

9. Submit Application for Entry Permit to DOA + copy of 10 day Health certificate, follow up!
ENTRY PERMIT will be send by (DO NOT ARRIVE WITHOUT PERMIT!)
10. Fly to Guam, BREATH, Contact Quarantine Facility, almost there!
Ones cleared for release, get license from DOA and now you can get your pet! A trip to the beach can be so healing – for both, you and your dog! Soak up some “Vitamin SEA” and relax! You all made it! Now off to house hunt! 
