7. On-campus Health Education Programme
Available for all students:
Oral health seminar (four times per year)
New students dental health screening programme (once per year)
To offer free dental checking for all freshmen once per year
Free screening for Orthodontics
Pre-employment dental services programme (once per year)
To provide free dental checking for graduates of the University
Free consultation for tooth whitening treatment

8. About applications
From 29 August 2016 (Mon), download the application form at SAO website (UGC students: here, Other students: here)
Submit the duly completed Application Form with Copy of Student ID card and bank-in slip/cheque for annual fee to SAO counter (B4-G/F-01) on or before 26 September 2016 (Mon)
After 26 September, post or fax the application documents to the service provider directly
The service provider will notify effective date of the membership through SMS, which normally start within 7 working days upon receiving the application
